Generally, when the VA issues a written decision, they will include a VA Form 21:09, which is labeled “Notice in Disagreement.” And they will include that in the packet with along with the rating decision that they're going to send the veteran or claimant. This is what the phone looks like in cases where a form is provided to the veteran: has to use that form to tell the VA that they're disagreeing with the decision. If so, the VA is going to be into the form. Otherwise, the VA is not going to accept any other disagreement on any other document, including any other VA forms. There are very few specific instances where a form is not provided by the VA.
